OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a pinch point hazard: An employee was performing a set-up operation on a Discmaster finish face grinder when he attempted to retrieve a part from the carrier by jogging the carrier forward. He mistakenly jogged it in reverse, pulling his finger into the guides and resulting in an amputation of one of his fingers. Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be to reduce this risk by applying the hazard control hierarchy as required by American National Standard Institute, ANSI, B11.9 - 2010 (R2015), Safety Requirements for Grinding Machines, Section 5, i.e. install a two-hand control versus relying on work practices when jogging the machine.
