29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    (a) Temporary service box, across the street from Lot #40: On or about April 20, 2016, and at times prior, the employer exposed employees to electrical shock hazards from a damaged extension cord, in that the internal electrical circuits were exposed as the protective casing and the insulation of the internal wires were damaged in the location near the strain relief at the male end of the 50-ft. extension cord.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.    (a) New residential construction, front side: On or about April 20, 2016, and at times prior, the employer exposed employees to fall hazards of approximately 13-feet, 6-inches from the second floor window while using a damaged ladder, in that the rungs were bent, the side rails were cut, and one of the foot pads was missing as employees used this ladder to access the upper level and failed to remove this ladder from service.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) New residential construction, second floor top plate: On or about April 20, 2016, and at times prior, the employee operating a pneumatic framing nail gun was exposed to eye injuries, in that protective eyewear was not provided while operating a pneumatic Hitachi (Model - NR83A) 3-1/4-inch Framing Nail Gun at 120 PSI.    Cesar Garcia was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1050194, citation number 1, item number 2 and was affirmed as a final order on 05/12/2015, with respect to a workplace located at 4669 Biscayne Circle, Powder Springs, GA 30127.

Cesar Garcia was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.304(d), which was contained in OSHA inspection number 1050194, citation number 1 , item number 2 and was affirmed as a final order on 05/12/2015, with respect to a workplace located at 4669 Biscayne Circle, Powder Springs, GA 30127.
