Soldered splices were not first spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:    Washer and Dryer Room, On or about April 22, 2016, and time prior thereto, employees where exposed to electrical hazards while using the pendant control on a dryer that had splices on the electrical conductor that were made with wire nuts and electrical tape.
