29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   (a)  On April 22, 2016, at the front of the Benson Guam store, fall protection was not provided for each employee working near the edge of the roof, exposing employees to the hazard of falling from a height of about 14 feet to the ground.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):   (a)  On April 22, 2016, employees working from elevations such as roofs were not trained in the recognition and the avoidance of unsafe conditions and the methods to minimize those hazards.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   (a)  On April 22, 2016, at the front of the Benson Guam store, an employee used an 8 foot fiberglass stepladder in the folded position to access the upper portion of the column.
