29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: On or before April 11, 2016 at the Bakery Equipment.com facility employees were being required to wear 3M 07193, dual cartridge respirators while performing painting operations and there was no written respiratory protection program. Employees were not provided a medical evaluation prior to being required to use the respirators.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or on dates prior to April 11, 2016 at the bakery equipment.com facility employees were exposed to serious amputation, crushing and struck-by injuries, in that the employer had not developed an Energy Control Program for the various types of machinery and equipment. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
