29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:   At the worksite located at 1605 S 212th Cir, Elkhorn, NE 68022, the employer is failing to protect employees from fall hazards:  Employees were observed working on a 4 in 12 pitched residential roof installing ice and water shield membrane without any type of fall protection system in use.  This exposed the employees to falls ranging between 12.5 and 21 feet from eave to ground.

29 CFR 1926.1053(b)(1):  Where a portable ladder was used for access to an upper level landing surface and the ladder's side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  At the worksite located at 1605 S 212th Cir, Elkhorn NE 68022 the employer is failing to protect employees from fall hazards:  An extension ladder used for access and egress to roof deck 12.5 feet above the ground did not extend at least 3 feet above the surface being accessed.  The ladder was also not secured at its top to a rigid support.
