29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion      (a) On or about 4/25/16 at Satya Twena LLC doing business as Makins Hats LLC located at 212 West 35th Street 12th floor, employees involved in the production and finishing of hats without gloves were exposed to hazardous chemicals including but not limited to toluene, hexane, and isopropyl alcohol. The employer failed to mandate the usage of gloves among employees exposed to hazardous chemicals.       NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:      (a) On or about 4/25/16 at Satya Twena LLC doing business as Makins Hats LLC located at 212 West 35th Street 12th floor, an employee working at the hat blocking station without face protection was exposed to chemicals including but not limited to 2-butanone, isopropyl alcohol, toluene, and hexane splashing into the face. The employer failed to assure that face protection was used by employees when spraying chemicals.       NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

The employer failed to make sure that containers were labeled with the appropriate pictograms (e.g., flammable liquid and respiratory hazard).    NOTE: THE EMPLOYER IS NOT REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM BECAUSE THE ITEM WAS ABATED DURING THE INSPECTION.
