29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Roof, Autumn Leaves Memory Care Center located at 3300 Webb Bridge Road, Alpharetta, GA 30005: Employees operating Hitachi (Model - NV45AB2) 1-3/4-inch Coil Roofing Nail Guns were exposed to eye injuries as protective eyewear was not provided. Employees using this equipment were exposed to struck-by hazards which included metal collated fragments, roofing particles and fasteners as safety glasses and/or goggles were not worn.

(b) Upper roof, Autumn Leaves Memory Care Center located at 3300 Webb Bridge Road, Alpharetta, GA 30005: Employees were exposed to falls of approximately 27-feet to the lower level while engaged in roofing activities on a 6/12 pitched roof without a fall protection system in place.
