29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): On or about April 25, 2016, a. an employee was exposed to fall hazards while performing roofing activities on the roof of the garage because of the lack of fall protection. The employee was exposed to a fall distance of 9.9 feet. b. an employee was exposed to fall hazards while performing roofing activities on the roof over the front porch because of the lack of fall protection. The employee was exposed to a fall distance of 9.7 feet. c. an employee was exposed to fall hazards while performing roofing activities on the roof of the house because of the lack of fall protection. The employee was exposed to a fall distance of 18.6 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about April 25, 2016, a. an employee was exposed to fall hazards while performing roofing activities on the roof of the garage at a height of 9.9 feet above the ground below due to the lack of fall protection and the employer did not have a training program for providing fall protection for the employee. b. an employee was exposed to fall hazards while performing roofing activities on the roof over the front porch at a height of 9.7 feet above the ground below due to the lack of fall protection and the employer did not have a training program for providing fall protection for the employee. c. an employee was exposed to fall hazards while performing roofing activities on the roof of the house at a height of 18.6 feet above the ground below due to the lack of fall protection and the employer did not have a training program for providing fall protection for the employee.
