29 CFR 1910.132(c): Design. All personal protective equipment shall be of safe design and construction for the work to be performed.  a. At the site located at 1164 Jaeger Street in Columbus, Ohio, employees were provided body belts for fall protection while working from a pole platform on a cross-connect box. Employees were working approximately twelve feet above the ground.  Body belts were not designed for fall protection.

29 CFR 1910.132(f)(1): The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following:    29 CFR 1910.132(f)(1)(iv): The limitations of the PPE    a. At the site located at 1164 Jaeger Street in Columbus, Ohio, employees were exposed to fall hazards and serious internal injuries while working on a cross-connect box. Employees had not been properly trained in the limitation of body belts while working off of the pole mounted aerial platform. Employees were provided with body belts that were to be used as personal fall arrest systems instead of their intended purpose as positioning devices.

29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;   a. At the workplace, the employer did not select and identify the types of PPE needed to protect employees accessing and working from aerial balconies. Employees were exposed to electrical and fall hazards while working from the pole mounted aerial balconies when they serviced cross-connect boxes.

29 CFR 1910.268(b)(6): Support structures. No employee, or any material or equipment, may be supported or permitted to be supported on any portion of a pole structure, platform, ladder, walkway or other elevated structure or aerial device unless the employer ensures that the support structure is first inspected by a competent person and it is determined to be adequately strong, in good working condition and properly secured in place.   a. At the site located at 1164 Jaeger Street in Columbus, Ohio, employees were exposed to an aerial balcony's platform that was not in good working condition in that it had rotted wooden decking and unsecured support members. The platform's wooden deck also had pieces that were broken off, leaving holes in the deck.  Employees stood on the pole mounted aerial balcony while performing cross-connect box servicing operations.
