29 CFR 1926.502(f)(3): No employee shall be allowed in the area between a roof edge and a warning line unless the employee is performing roofing work in that area. a) Roof at 5210 Jaindle Blvd. Bethlehem, PA - on or about April 27, 2016, a safety monitor not performing roofing work was positioned between the warning line and unprotected edge, exposed to fall hazards greater than six feet.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units: a) 5210 Jaindle Blvd. Bethlehem, PA - on or about April 27, 2016, employees were using a JLG 500 RTS Scizzor scissor lift where two of the outrigger pads were cribbed with various sized materials, which were unevenly stacked and did not support the full circumference of the individual pads.
