29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as used as a means of access.  (a) Front of the building - On or about April 22, 2016 employees were exposed to a fall hazard while working on a scaffold 13.8 feet above the ground with a ladder that did reach the top platform. The ladder was 12.2 feet long and only reached the 2nd level of the scaffold.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  (a)  Front of the building - On or about April 22, 2016 employees were exposed to a fall hazard while working on a scaffold 13.8 feet above the ground with incomplete guardrails installed. The guardrails were missing in several sections of the scaffold.
