Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. a. Located at the north wall exit to the loading docks in the warehouse: On or about April 27, 2016, the working space in front of the 120/208 volt electrical panel, which fed the forklift battery charger and part of the warehouse lighting, was obstructed by a battery charger. b. Located on the wall outside the office/break room in the warehouse: On or about April 27, 2016, the working space in front of the 120/208 volt electrical panel, Electrical Panel E, was obstructed by stored equipment, such as a pallet jack and a stand up desk. c. Located on the north wall of the High End area in the warehouse: On or about April 27, 2016, the access to and working space at the 120/208 volt electrical panel, Electrical Panel A, was blocked by skids of product.

On April 27, 2016, an OSHA compliance officer requested the employers OSHA 300 and 300A injury and illness logs during an inspection on-site. The employer failed to provide the logs to OSHA within four hours.
