29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed:    a) Building one: A chain and sprocket that operated the large rolling machine was not guarded, on or about 4/27/16.    b) Building two: A chain and sprocket that operated the stencil machine was not guarded, on or about 4/27/16.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.         a) Facility: The employer did not report an amputation of an employee's middle finger after her hand was pulled into a rolling machine, within the required 24 hours, on or about 7/3/15.    Aluminum Shapes LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2),  which was contained in OSHA inspection number 1120619, citation number 1, item number 1 and was affirmed as a final order on 3/24/16 with respect to a workplace located at 9000 River Road Delair, NJ 08110.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Building one:  On 4/22/16 an employee's hand was pulled into the guard on a large roll forming machine that partially covered the point of operation and had an opening of 12 inches. The employee suffered an amputation of the tip of the  index finger.     b) Building two: A guard on a small roll forming machine had an opening of six inches exposing employees to the point of operation, on or about 4/27/16.    c) Building two: A Uhall stenciling machine had unguarded rollers exposing employees to the point of operation, on or about 4/27/16.      Aluminum Shapes LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 315978676, citation number 1, item number 4, and was affirmed as a final order on 5/3/12, with respect to a workplace located at 9000 River Road Delair, NJ 08110     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
