29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about April 27, 2016, employees performing residential roofing operations were exposed to struck by hazards while employees handed plywood sheets to the second level from the first level. The employer failed to mandate the use of hard hats. Abatement certification is required of this item in accordance with requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about April 27, 2016, employees performing residential roofing operations were exposed to a 19.8 foot fall when the employer failed to ensure that fall protection was utilized. The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 987924, citation number 1, item number 1 and was affirmed as a final order on September 5, 2014, with respect to a workplace located at 25224 Parkside Drive, Lot 1145, Plainfield, Illinois 60544. The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number719138, citation number 1, item number 2 and was affirmed as a final order on December 12, 2012, with respect to a workplace located at 4528 Cork Tree Lane, Naperville, Illinois 60564. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
