29 CFR 1926.152(d)(4): At least one portable fire extinguisher having a rating of not less than 20B:C units was not provided on all vehicles used for transporting and/or dispensing flammable or combustible liquids: The employer did not ensure that a portable fire extinguisher was available to employees who were carrying a 5 gallon gasoline can in the vehicle. The gasoline was being used to fill the generator.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: The employer did not ensure fall protection was implemented for employees performing residential roofing operations who were exposed to the hazard of falling approximately 21.9 feet. Comfort Livings was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 993638, citation number 1, item number 1a and was affirmed as a final order on 2/3/2015, with respect to a workplace located at 1400 Mexico Rd., O'Fallon, MO 63366.
