29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    The employer did not ensure that employees, who were performing roofing operations, were protected from falling through holes (including skylights) on walking/working surfaces.    NOTE:  29 CFR 1926.502(i)(2)              Covers for holes in floors, roofs, and other walking/working surfaces shall meet the following               requirements: all other covers shall be capable of supporting, without failure, at least twice the               weight of employees, equipment, and materials that may be imposed on the cover at any one time.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems:      The employer did not ensure appropriate fall protection was implemented for employees performing commercial roofing operations on a 4:12 pitch roof that was greater than 50 feet wide when they were exposed fall hazards associated with an approximately twenty (20) foot fall.    The employer (Kory Dunnavant dba Comfort Living) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1143639, citation number 2, item number 1 and was affirmed as a final order on 06/22/2016, with respect to a workplace located at 2881 Brookmeadow Dr., Belleville, IL  62221.    The employer (Kory Dunnavant dba Comfort Living) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 897120, citation number 1, item number 2b and was affirmed as a final order on 05/01/2013, with respect to a workplace located at 2460 and 2462 Delmar Ave, Granite City, IL  62040.
