29 CFR 1926.1101(k)(3)(ii): Before work under this standard was performed, the employer did not inform employees of the location and quantity of ACM and/or PACM present in the area and the precautions to be taken to insure that airborne asbestos is confined to the area. Floors 22,24,25 and 26 On or about 3/28/16, employees performing demolition work were not told where asbestos containing materials or presumed asbestos containing materials were located in the area they were working. Black mastic and associated floor tiles in the freight elevator vestibules on all floors and on the south stairwell landings for all floors contained 3% chrysotile asbestos.

29 CFR 1926.1101(k)(9)(vi): Training for employees performing Class IV operations did not include available information concerning the locations of thermal system insulation and surfacing asbestos containing material and/or presumed asbestos containing material, asbestos-containing flooring materials, or flooring material where the absence of asbestos has not yet been certified; and instruction in recognition of damage, deterioration, and delamination of asbestos containing building materials; and did not take at least 2 hours: Floors 22,24,25 and 26 On or about 3/28/16, employees performing demolition work in an area that contained asbestos containing material and material that had not been tested, were not provided training, including the location of such materials and recognition of damage.
