29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) At 2118 Plaza Court in St. Charles, MO 63303 - On or about 04/28/2016, the employer failed to provide, and require the use of, eye protection for employees exposed to the hazard of being struck by flying objects while operating pneumatic nailers.   The (Carlos Martinez) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.102(a)(1)), which was contained in OSHA inspection number 315091843, citation number 1, item number 1 and was affirmed as a final order on (07/24/2015), with respect to a workplace located at 2117 S 279th W. in Garden Plain, KS 67050.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At 2118 Plaza Court in St. Charles, MO 63303 - On or about 04/28/2016, the employer failed to provide, and require the use of, fall protection for each employee working on a steep residential roof (5-in-12 pitch or greater) approximately 20 feet above a concrete surface.    The (Carlos Martinez) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 315091843, citation number 1, item number 2 and was affirmed as a final order on (07/24/2015), with respect to a workplace located at 2117 S 279th W. in Garden Plain, KS 67050.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 2118 Plaza Court in St. Charles, MO 63303 - On or about 04/28/2016, employees were exposed to elevated fall hazards where a portable ladder used to access the roof and the ladders length allowed, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed.    The (Carlos Martinez) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.1053(b)(1)), which was contained in OSHA inspection number 315091843, citation number 1, item number 3 and was affirmed as a final order on (07/24/2015), with respect to a workplace located at 2117 S 279th W. in Garden Plain, KS 67050.
