29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:  a. Site- Employees accessing a ladder jack scaffold were not protected from a fall hazard where a portable ladder was not provided to access the scaffold platform. Employees were using the ladders supporting the scaffold platform to access the scaffold by climbing over the scaffold platform.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a. Site- Employees using an aluminum extension ladder to access a ladder jack scaffold platform were not protected from a fall hazard where the ladder extended less than one rung above the landing surface. The ladder was fully extended and did not have a grasping device provided to assist in mounting and dismounting the ladder.

Site- Employees using an aluminum extension ladder to access a ladder jack scaffold were not protected from a trip hazard where the ladder erected on top of a wood pallet and bundles of shingles. Also there was debris and pneumatic hoses around the base of the ladder creating a trip hazard as employees mounted and dismounted the ladder.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  a. Site- Employees working from the roof and ladder jack scaffolding were not protected from a fall of approximately 25 feet while using personal fall arrest equipment. Employees wearing full body harnesses which were connected to lifelines with lanyards did not have adequate anchor points to arrest a fall. Lifelines for personal fall arrest systems where anchored to the peak of the roof by having a single nail driven through the core of the rope and into the ridge beam of the roof.
