Site - On or about March 9, 2018, employees accessing a ladder jack scaffolding were not protected from a fall hazard where a portable ladder was not provided to access the scaffolding platform. Employees were using the ladders supporting the scaffolding platform to access the scaffold by climbing over the scaffold platform.
