Employees are required to retrieve and stock materials from the metal storage racks. Employees are climbing and standing onto the metal storage racks approximately 6-20ft. above ground level, to retrieve storage items, without utilizing fall protection, on or about 05/03/16.

Employees were using the 8ft. self-supporting A-frame ladders as a extension ladder to retrieve storage materials from the storage racks. The ladders were not used in the manner that the manufacture intended, on or about 05/03/16.

Flexible extension cord was used as a replacement for permanent wiring running to the overhead lighting fixtures. throughout the building. The flexible extension cords were not in a metal conduit to prevent outer insulation damage, on or about 05/03/13.
