29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) On or about 05/03/16, on the north side of the upper roof of a house at 95 Crossman Ave.,Buffalo, NY:  Four employees engaged in roofing  tearoff work on a steep roof  were exposed to a fall of  14 foot  1 inch.  No fall protection system was utilized. Pitch of the roof was 8 in 12.   b) On or about 05/03/16, on the south side of the upper roof of a house at 95 Crossman Ave.,Buffalo, NY:  Two employees engaged in roofing  work on a steep roof  were exposed to a fall of  14 foot  1 inch.  No fall protection system was utilized.  Pitch of the roof was 8 in 12.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 05/05/16, on the north side of the upper roof of a house at 95 Crossman Ave., Buffalo, NY:  Employee accessed the roof by using an extension ladder. The employee was exposed to a potential fall of up to 14 ft.  because the siderails of the ladder only extended approx. 18 inches above the roof.    NO ABATEMENT CERTIFICATION REQUIRED
