29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    Worksite: In the following instances employees performing roofing work were exposed to eye and face injuries and protective equipment was not supplied to reduce the or eliminate the potential for injury:  (a) On or about 5/4/16, employees operating pneumatic nail guns were exposed to eye injuries where eye protection was not worn or provided;  (b) On or about 5/6/16, an employee operating a gasoline-powered leaf blower to remove dirt, wood debris, and loose roofing nails on a roof was not wearing personal protective equipment to prevent eye and face injuries.  In addition, other Roofers in close proximity were not given eye and face protection to protect them from being struck by the same debris.

29 CFR 1926.501(b)(13): The employer does not protect each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels protected from falling by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    This violation was most recently observed on 05/04/2016 - 05/12/2016 at a worksite at 55 Rodgers Lane in Middletown RI (main roof, roof over entrance, roof over laundry center) where adequate fall arrest systems were not provided, exposing employees working on the roofs to a fall of 9 1/2 to 18 feet.    To abate this violation, the employer must ensure that a fall protection system is in place to protect each employee working 6 feet or more above a lower level from falling.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface is not protected from tripping in or stepping into or through holes (including skylights) by covers:    This violation was most recently observed on 05/06/2016 at a worksite at 55 Rodgers Lane in Middletown RI  where employees were working in the vicinity of an open hole in the roof, and exposing these employees to trip and fall hazards.    To abate this violation, the employer must ensure that holes are protected by an adequate guard or cover before allowing work to proceed.    M & M Roofing LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(4)(ii), which was contained in OSHA inspection number 985099, citation number 1, item number 2 and was affirmed as a final order on 10-21-14, with respect to a workplace located at 89 Flintlock Rd in Watertown CT.

29 CFR 1926.503(a)(1): The employer does not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    This violation was most recently observed on 05/04/2016 - 05/12/2016 at a worksite at 55 Rodgers Lane in Middletown RI where employees performing roofing work at heights of 9 1/2 to 18 feet above the ground and exposed to falling were not provided a training program to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards, such as but not limited to: proper procedures for setting up a personal fall arrest system, procedures for donning full body harnesses, and procedures for using fall protection lanyards and fall arrestors.    To abate this violation, the employer must ensure that all employees exposed to a fall hazard are provided with training before allowing work to proceed.    M & M Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 315555771, citation number 1, item number 1 and was affirmed as a final order on 9-6-11, with respect to a workplace located at 31 Todd Rd in Walcott CT.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    This violation was most recently observed on 05/04/2016 at a worksite at 55 Rodgers Lane in Middletown RI  where employees were using a ladder to access the work area on the roof, and the ladder did not extend at least three feet above the upper landing surface of the roof.    To abate this violation, the employer must ensure that all ladders on the jobsite are properly set up so the side rails extend at least 3 feet above the upper landing surface of the work area being accessed.    M & M Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1026.1053(b)(1), which was contained in OSHA inspection number 902477, citation number 1, item number 3 and was affirmed as a final order on 2-13-14, with respect to a workplace located at 215 Tudor Lane in Manchester CT.
