OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees where employees were exposed to heat exhaustion, entrapment and general suffocation within a smoke house: On or about May 4, 2016, at smoke house number 4, the walk-in smoke house door would not open from the inside. The internal quick release mechanism was sticking and employees routinely enter the space.

Employees must serpentine and go through approximately four rooms to get outside from the smokehouse department. Employees exposed to the hazard of smoke inhalation or burns.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about May 4, 2016, in the wash bay area, the employees were using chemicals that were corrosive to the eyes and skin. The path to the eyewash station is obstructed and employees must go through a door to get to the eyewash station from the storage room. Employees exposed to the hazard of chemical burns to the eyes and skin.
