29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about May 8, 2017, at the Kleenline, there were no energy control procedures developed or utilized. An employee's wrist was caught in the machine while attempting to adjust the belt on the conveyor. The Kleenline has a minimum of electrical and pneumatic energies. Employees exposed to the hazard of being caught-in the machine.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: On or about May 8, 2017, the employer failed to ensure the employees were trained and they had the knowledge and skills for the safe application, usage and removal of energy control devices when maintaining, cleaning, unjamming, and the adjustment of multiple energy source equipment such as the Kleenline. Employee was adjusting the Kleenline with the guards off and machine running and got caught-in the machine. Employees exposed to a caught-in hazard.
