(b) Facility- 5441 National Pike, Markleysburg, PA: On or about May 4, 2016, employees working on the grading deck in the scrag mill were exposed to fall hazards of approximately 80".

Employees were observed locking out equipment such as but not limited to the Stac Trac to conduct maintenance activities.
