29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:    a) In the facility, on or about June 16, 2016 - An employee designated as a Lumber Grader was exposed to continuous noise levels at 233% of the allowable 8-hour time-weighted average sound level (90 dBA).  The equivalent dBA level of the 233% dose is approximately 96.1 dBA.  The sampling was performed for 474-minutes during the first shift on June 16, 2016.  Zero exposure was assumed for any unsampled time.  Feasible administrative or engineering controls were not utilized to reduce sound levels.    b) In the facility, on or about June 16, 2016 - An employee designated as an Edger Operator was exposed to continuous noise levels at 563% of the allowable 8-hour time-weighted average sound level (90 dBA).  The equivalent dBA level of the 563% dose is approximately 102.5 dBA.  The sampling was performed for 476-minutes during the first shift on June 16, 2016.  Zero exposure was assumed for any unsampled time.  Feasible administrative or engineering controls were not utilized to reduce sound levels.

The Volcollect single earcup headset worn by the employee over the right ear provides a NRR of 10 and an adjusted attenuation of 1.5 utilizing a safety factor of 50%.  The noise received by the employee's right ear was calculated to be 94.6 dBA and is not sufficient to meet the requirements of Table G-16.
