29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:    a) On or about 5/4/2016, throughout the plant, the employer allowed employees to access and work near ladderway floor openings in order to clean and check on equipment and did not ensure that swinging gates were provided on openings whose passage through the railing was not offset.     b) On or about 5/4/2016, on the entoleter platform, the employer allowed employees to access the platform and did not ensure that toeboards were provided on exposed sides of the ladderway opening.
