29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 28, 2016, at the National Gypsum, Inc. Westwego plant, signs were not provided for operators working on the dry end panel conveyor reminding them that equipment must be locked out before attempting to unjam toed rollers.

On or about May 28, 2016, at the National Gypsum, Inc. Westwego, LA plant, the employer failed to ensure that all personnel working on or near a conveyor were instructed in the location and operation of all stopping devices.

29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year. National Gypsum, Inc. did not record the following workplace injury correctly or completely on the OSHA 300 Log for calendar year 2015: a) The employer does not ensure that entries on the OSHA 300 log are complete, accurate and corrections made to any deficiencies identified. The violation occurred on or about January 15, 2016, when an injury that occurred on September 14, 2015, was not included on the OSHA 300 log for 2015. The employee suffered a dislocated toe while operating a pallet lift in reverse.
