Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while working from order pickers without using body belts. On May 4, 2016 the CSHO found that the employer did not ensure that employees, who were pulling stock onto elevated platforms and loading shelves, were using fall protection.

Fall from the mezzanine by the loading docks - approximately 8 feet 6 inches b. Fall from the mezzanine in the back by the battery charger - approximately 13 feet 10 inches

Front of the warehouse by the loading dock - both the egress door and adjacent equipment door were blocked by boxes and pallets of plumbing supplies. b. Across from the service desk, left end of the dock looking out, was blocked by pallets and boxes of plumbing supplies. c. Rear of the warehouse by rack D032 was blocked by storage of plumbing supplies and shelving equipment.

Supply New England, Warehouse, 582 Quaker Highway, Uxbridge, Massachusetts; On May 4, 2016 powered industrial truck damage to racks in the warehouse was found. There was no indication of driver retraining for employees involved in near misses and accidents while driving powered industrial trucks.
