29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) On or about May 4, 2016, Facilities Contracting, Inc., did not ensure that employees of R. J. Gleeson, subcontractor, were protected from cave-in hazards while they were working in a trench that was 6.9 feet deep, was 40 feet long, and had no means of egress at the east end. The west end of the excavation had a sloped ramp for employees to enter and exit, but the east end had no means of entrance and egress. A ladder was on the spoil pile on the north side of the trench. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

J. Gleeson, subcontractor, were protected from cave-in hazards while working in a trench in which water was standing and accumulating. The trench had no protective system nor precautions taken to mitigate the water accumulation.
