29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about May 02, 2016 and at times prior thereto, employees were working on a rig site, Nomac Drilling Rig #56. There were three instances were extension cords were connected together: One had three extension cords connected together from the septic pump to the superintendent trailer and the other two had four extension cords connected together from the septic pump to the two crew trailers. This exposed employees to fire and electrical shock hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about May 02, 2016 and at times prior thereto, employees were working on a rig site, Nomac Drilling Rig #56. The cord used to power the mud logger trailer had some strain relief damage. This condition exposed employees to electrical shock hazards.
