29 CFR 1910.27(f): Ladders were not maintained in a safe condition or inspected regularly: On or about May 03, 2016 and at times prior thereto, employees were working on a rig site, NorAm Rig #25. On the rig floor, the derrick ladder was not secured properly and was bent in an awkward position. This exposed employees to fall hazards.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors: On or about May 03, 2016 and at times prior thereto, employees were working at a rig site, NorAm Rig #25. At the change quarters trailer, a 240 V cable was run through a hole in the wall. The cord had an indentation already on it. This exposed employees to fire and/or electric shock hazard.
