29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Drill site: On or about May 2, 2016 and at times prior thereto, the strain relief was broken on the power cord going into the breaker box. This condition exposed the employees to electrical shock hazards.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: Drill site: On or about May 2, 2016 and at times prior thereto, the electrical cord was not protected when run through the breaker box opening. This condition exposed the employees in two instances to electrical shock hazards.
