29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Drilling Site: On or about November 14, 2017, employees were exposed to electrical shock hazards due to the employer not grounding two trailers to earth in the following instances. a) SLS#41 trailer (Directional Driller's trailer) ground cable was wrapped around the trailer frame. b) BSS#9 trailer (Emergency shower & bathroom) was not grounded.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: Drill site: On or about November 14, 2017, trailer SLRS AT #12 circuit breaker box has one flexible cord and a solid conductor not protected from abrasions when run through breaker box opening. This condition exposed employees to electrical shock hazards.
