29 CFR 1910.146(f): The entry permit that documented compliance with this section and authorized entry into a permit space did not identify the requirements in (f)(1)-(15) of this section: (a) Boiler Room - On or about May 13, 2016 the employer's written confined space program does not sufficiently meet the requirements of 29 CFR 1910.146(f) in that it does not identify (f)(2) the purpose of the entry; (f)(3) duration of the permit; (f)(5) the individuals serving as entry attendants; (f)(7) the hazards of the permit space to be entered; (f)(8) measures used to isolate the permit space and to eliminate or control permit space hazards before entry; (f)(9) the acceptable entry conditions; (f)(10) results of initial and periodic test performed under paragraph (d)(5) of this section, accompanied by the names or initials of the testers and by an indication of when the tests were performed; (f)(11) the rescue and emergency services that can be summoned and the means for summoning those services; (f)(12) the communication procedures used by authorized entrants and attendants to maintain contact during the entry; (f)(13) equipment, such as personal protective equipment , testing equipment, communications equipment, alarm systems, and rescue equipment, to be provided; (f)(14) other information necessary given the circumstances of the particular confined space, in order to ensure employee safety; and (f)(15) any additional permits, such as for hot work, that have been issued to authorize work in the permit space.
