29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a)  On or about 05/06/2016 - Shipping Office Trailer; Heating/cooling vents and work surfaces, including but not limited to, desks and office equipment, were visibly coated with a thick layer of dust/dirt that contained 165 to 397 micrograms of iron oxide. Iron Oxide is a component of Black Beauty and Red Primer which are used in the Painting and Lining Departments located in the air intake side of the Shipping Department trailer's AC unit.        b)  On or about 05/06/2016 -- Connex Boxes;  Pallets, parts and other equipment accessories were located in front of passageways (entry doors) into the Connex storage boxes created tripping hazards for employees entering and exiting the boxes to obtain parts and accessories.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purpose, the load approved by the building official was not appropriately marked and posted:    a)  On or about 05/06/2016 - Used Connex storage boxes; Parts and equipment were being stored on top of the used Connex boxes that employees entered into frequently to procure parts. The load rating for the roof of the boxes was not posted.  Connex boxes 4, 7 and 10 had materials stored on top and their roofs were bowed. Connex box #10 had one roof support rail that had rusted and was disconnected on one end.  Several other rail connections in Box 10 were also rusted.

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:    a)  On or about 05/17/2016 - Black Beauty Blasting;  An annual audiogram was not provided for an employee performing black beauty blasting operations who was exposed to continuous noise at a level of 621% of the allowable 8-hour time weighted average sound level of 90 dBA or an equivalent sound level of 103 dBA during a sampling period of 409 minutes.  Exposure calculation included a zero increment for the 71 minute time period not sampled.            b)  On or about 5/17/2016 - River Sandblasting;  An annual audiogram was not provided for an employee performing sandblasting operations who was exposed to continuous noise at a level of 299% of the allowable 8-hour time weighted average sound level of 90 dBA or an equivalent sound level of 98 dBA during a sampling period of 426 minutes.  Exposure calculation included a zero increment for the 54 minute time period not sampled.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:     (a)  On or about 05/17/2016 - River Sandblasting;  A medical evaluation to determine that a temporary employee was medically able to wear a respirator was not provided by the employer prior to requiring the temporary worker  to wear a Bullard blasting hood while conducting sandblasting operations.  The temporary employee was working in the adjacent to the blasting area where a sandblaster was sampled for exposure to silica on 05/17/2016.  The measured silica exposure for the 421 sample taken on 05/17/2016 was determined to be 13.15 mg/m3 which was 36 times the permissible exposure limit of 0.36 (based on sample containing 26% crystalline silica).  Exposure calculation included a zero increment for the 59 minute period not sampled.

C & B Piping, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134 (g)(1)(i)(A), which was contained in OSHA inspection number 68859, citation number 1, item number 1 and was affirmed as a final order on 12/28/2012 with respect to a workplace located at 8804 Parkway Drive, Leeds, Alabama.
