OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were operating overhead underhung hoists that had not been inspected or maintained properly: a) On or about 06/01/2016 - Test bay unit #2, 2-Ton CM UR SG PTECH underhung hoist, the lower hook was stretched, and the safety latch was broken. b) On or about 06/01/2016 - Fabrication shop, 1-Ton CM URSGECH underhung hoist, the support connection had an anchor bolt missing, and there were two different sized anchors.
