29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) plus 4 inches (10 cm) for every 10 kV over 50 kV: On or about May 2, 2016, the employer failed to ensure that the Sennebogen 850 mobile material handler was not elevated near overhead electrical lines. The electrical services located at 3455 Campbell Road in Cleveland are supplied from a 3-PH, 4,340 V (line-to line), Delta System, overhead wood pole line. The equipment was operated in an elevated position and made contact with a line.
