29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 2, 2019, the employer failed to ensure that the material hopper and the box-lid on the D&J Press Co. Inc. Metal Briquetter Machine (model # 15-AR) were adequately guarded.

29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii): On or about December 5, 2019, the employer failed to ensure that gears were adequately guarding in order to prevent employees from being injured by equipment. The gears for the release shoot on the D&J Press Co. Inc. Metal Briquetter Machine (model # 15-AR) were not adequately guarded.
