29 CFR 1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations. Additionally, the engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure. (a) The employer did not ensure that an engineering survey was completed in writing that described, at minimum, the conditions of the framing floors and walls and any unplanned collapse.

29 CFR 1926.859(g): During demolition, continuing inspections by a competent person shall be made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. No employee shall be permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means. (a) The employer did not ensure that a competent person conducted continuing inspections during the demolition process to ensure that sufficient support for the excavator was provided at all times.
