29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  On or about May 11, 2016, the employer did not provide training to employees, upon initial employment, on portable fire extinguishers when available for employee use.   The Training Range, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(g)(2), which was contained in OSHA inspection number 1098243, citation number 1, item number 1 and was affirmed as a final order on December 16, 2015, with respect to a workplace located at 4445 Mahoning Ave. Youngstown, OH 44515.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:  On or about May 11, 2016, wipe samples were taken from surfaces in the gun range and cash register area. Analytical results indicated the presence of lead, as follows:    a) Lane 7 Table Top: 2123.0 ug/ft2 b) Lane 2 Table Top: 465.8 ug/ft2  Surfaces were not maintained as free as practicable from accumulations of lead.   The Training Range, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1025(h)(1), which was contained in OSHA inspection number 1098243, citation number 1, item number 2(b) and was affirmed as a final order on December 16, 2015, with respect to a workplace located at 4445 Mahoning Ave. Youngstown, OH 44515.

29 CFR 1910.1025(l)(1)(ii): The employer did not train each employee who is subject to exposure to lead at or above the action level, or for whom the possibility of skin or eye irritation exists, in accordance with the requirements of 29 CFR 1910.1025:  On or about May 11, 2016, company employees were exposed to lead while working and cleaning the shooting range. Employees have not received training on the hazards of lead.   The Training Range, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1025(l)(1)(ii), which was contained in OSHA inspection number 1098243, citation number 1, item number 2c and was affirmed as a final order on December 16, 2015, with respect to a workplace located at 4445 Mahoning Ave. Youngstown, OH 44515.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  On or about May 11, 2016, the employer did not provide employees with Appendix D of the respiratory standard when voluntarily using N-95 filtering face piece respirators while cleaning up the gun range.
