29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and addressing at least steps for each operating phase as outlined by this paragraph: A) Ross Industries BLC Freezer, written operating procedures were not developed for all operating scenarios including, but not limited to: Changes in production operation resulting from a switch between Automatic mode and Remote mode as managed by maintenance; normal shut down by any user; shut down for maintenance; shut down and energy isolation for cleaning; cleaning with conveyor or fans running; cleaning involving the Defrost Cycle; and re-start after Emergency Stop actuation by any user, on or about June 3, 2016. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
