29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about May 10, 2016 the point of operation on the Reed powered roller was not guarded to prevent employee exposure to the in-running nip point hazard on the back-side of the roller. b) On or about June 9, 2016 the point of operation on the Amada HDS 8025 NT was not guarded to prevent employee exposure to the hazardous downward movement of the top ram onto the die bed. c) On or about June 9, 2016 the point of operation on the Amada HDS 2204 NT was not guarded to prevent employee exposure to the hazardous downward movement of the top ram onto the die bed. d) On or about June 9, 2016 the point of operation on the Amada HFE 170 4S was not guarded to prevent employee exposure to the hazardous downward movement of the top ram onto the die bed.
