29 CFR 1910.146(d): Under the permit space program required by paragraph (c)(4) of this section the employer did not ensure that the requirements of (d)(1) through (d)(14) were met: On or about May 05, 2016, the employer's written confined space program did not describe and explain how all of the following required elements would be met: 1. Providing rescue services or training employees to use rescue equipment to ensure safe entry into and rescue from permit spaces; 2. Designating the persons who have active roles in entry operations, identify duties and providing adequate to conduct role required for entry; 3. Reviewing and updating the permit space program annually and as necessary to ensure employees entering spaces are protected from permit space hazards. ABATEMENT DOCUMENTATION REQUIRED
