29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Main entry way corridor; on or about 11 May 2016 and times prior thereto; a mobile vendor kiosk had a damaged electrical plug. Electrical plug was missing pins. This condition exposed employees to an electrical shock and electrical burn hazards.

Loading dock receiving area, desk one; on or about 11 May 2016 and times prior thereto; a four receptacle energized electrical outlet box was separated from the conduit exposing wires. This condition exposed employees to an electrical shock and electrical burn hazards.   The Army Air Force Exchanges Services was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 1136291, citation number 1, item number 2 and was affirmed as a final order on 04 May 2016, with respect to a workplace located at the Eielson Air Force Exchange Services Main Exchange.
