Power tap was plug into and energized outlet.         b. Receiving area; a 25 foot orange extension cord was missing the ground pin.  This condition exposed employees to electrical shock and electrical burn hazards.

Building 881; on or about 12 May 2016 and times prior thereto; the employer did not ensure that the electrical panels boards were readily accessible. Four instances were noted:      (a) Printer room; had a file drawer and miscellaneous items stored in front of electrical panel board impeding access.        (b) Bay 2 area; had roller cart with boxes stored in front of the electrical panel board impeding immediate and clear access.      (c) East bay, Panel labeled 6B; had a disassembled pallet rack stored in front of the electrical panel board impeding access.     (d) Mattress storage area; had mattress stored in from of the electrical panel board impeding access.  This condition exposed employees to an electrical shock hazard and burn hazard.   The Army Air Force Exchanges Services was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.304(f)(1)(iv), which was contained in OSHA inspection number 1136291, citation number 1, item number 2 and was affirmed as a final order on 04 May 2016, with respect to a workplace located at the Eielson Air Force Exchange Services Main Exchange.
