29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a) 1090 Springfield Road, Union, Union, NJ 07083 - Manufacturing:     The employer did not establish an Energy Control program that defines machine specific energy control procedures, training and periodic inspections for employees performing part change out, maintenance and/or servicing machines to prevent accidental energizing of the machines.     Machinery including, but not limited to, the Tetra Pak Cream machine did not have written procedures for de-energizing, and isolating energy sources.     Condition noted on or about 5/17/2016.

29 CFR 1904.39(a):  An oral or written report of an employment accident resulting in an employee being admitted to the hospital was not made within 24 hours after the occurrence to the nearest Area Office of the Occupational Safety and Health Administration (OSHA) and/or a call to the OSHA toll-free central telephone number 1-800-321-OSHA (6742):    a) 1090 Springfield Road, Union, NJ worksite:     The employer did not notify OSHA within 24 hours of an incident that occurred on/or about 05/12/2016 at the site and that resulted in an employee receiving an amputation injury.    Condition noted on/or about 05/17/2016.
