A) Deep Foods at 1090 Springfield Rd., Union, NJ 07083 -Douglas Cartoner (Serial # M-110004, Model # WA-IM) - The employer failed to conduct annual periodic inspections for all authorized employees following the procedures and the requirements of the energy control program. This condition exposed employees to uncontrolled hazardous energy. Hazards observed on or about June 17, 2025

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips. A) Deep Foods/Douglas Cartoner (Serial # M-110004, Model # WA-IM) at 1090 Springfield Rd., Union, NJ 07083 - 1. The employer did not have adequate guarding to prevent employees from approaching pinch points at the discharge belt. 2. Door #5 was equipped with an interlock switch that was covered with tape preventing it from de-energizing the equipment when the door was opened. This condition exposed employees to amputation hazards. Incident occurred on June 17, 2025
